What were you doing: Killing Hydra boss to complete my obsidian badge
What happened: around 10% of all times when i tried to reset the boss it says that it has been reset, however cursed skulls didn't appear and the timer reset tho
What did you expect to happen: no problems with spawning
How recently did this happen: as it was said above - around 10% of all times 
Do you have any screenshots or videos? Image (here you see that cursed skulls didn't appear while message of its resetting has appeared and no boss parts are presented to indicate that i'm just lying)

There's also this issue of clouds 'timelapsing' when moving around (sometimes on a mount), i.e. they suddenly jerk forward for no apparent reason
